  • Responsibilities
    1. Coordinates program components to include staff and community education, patient education, quality metric compliance monitoring and program needs assessment evaluation. Collaborates with multidisciplinary team and coordinates delivery of plan of care.
    2. Responsible for planning, implementing and coordinating a standardized method of delivering clinical care.
    3. Coordinate the performance improvement process, program development, planning, organizing, implementation, and monitoring.
    4. Coordinate education for physicians, clinical and ancillary staff, patients, and community outreach.
    5. Ongoing patient tracking and concurrent and retrospective chart abstraction for patient monitoring and improvement opportunities.
    6. Knowledge of regulatory requirements for Trauma Certification and Acute Stroke Ready programs.
    7. Strong management skills that include organizational, analytical verbal and written communication competencies and an orientation to detail.
    8. Quality representative - responsible for ensuring that the requirements of the program are implemented and maintained.
    9. Clinical activities - promotes the delivery of evidenced based care, clinical resource, guideline development.
    10. Education - coordinate intra facility and regional educational activities.
    11. Performance improvement - monitors clinical and system processes develop filters and case review indicators, identifies trends and assist with remedial needs. Runs reports as needed.
    12. Liaison - acts as liaison to local agencies, administration, physicians and healthcare staff.
    13. Supervision of registry database. Maintains accurate registry and uploads data as required.
    14. Serves as the Stroke Coordinator for McLeod Medical Center Dillon.
    15. Manages program budget.

    QUALIFICATIONS/TRAINING:

    • Five years’ experience in related nursing area.

    LICENSES/CERTIFICATIONS/REGISTRATIONS/EDUCATION

    • Bachelor's Degree (BSN) in nursing
    • Must possess a valid SC license- RN, BLS

    WORK SCHEDULE:

    • Full-time, 80 hrs bi-weekly
