    Director of Post-Acute Care Operations

     

    Job Summary:

    Reporting to the Corporate Vice President of Case Management, this position provides the operational leadership to improve business unit performance and sustainability in support of patient access to and ongoing care in the post-acute care environment. It is aligned closely with the Case Management division. The position oversees a mix of operational and business activities for Swing Bed programs as well as working with McLeod Health preferred post-acute providers. Work is closely aligned with hospital acute care providers and Facility Medical Directors to ensure smooth transition of care as patients move through the continuum. This position requires full understanding and active participation in fulfilling the Mission of McLeod Health and the specific Market/s it supports. This employee shall support overall strategic plans, goals, and direction of the quality improvement /process improvement activities. 

    Essential Job Functions:

    1. Oversees a mix of operational, and business activities translating required program elements to operations. This shall include supervision and management of staff who carry out initiatives promoting safe coordinated transitions of care as patients move throughout the care continuum.
    2. Assembles and manages program resources, including articulation of objectives, roles, and expected results.
    3. Develops corporate operations guidelines by writing and updating standard operating procedures.
    4. Maintains operations by initiating, coordinating, and enforcing program, operational, and personnel policies and procedures.
    5. Utilizes and interprets data to identify factors allowing for rapid change/course adjustment to stay on target and achieve operational and program goals.
    6. Understands and is able to implement rapid cycles of change in response to changing episode payment models and value based care.
    7. Maintains documentation of program operations by auditing work effort of Post-Acute Care Directors.
    8. Maintains customer confidence and protects operations by monitoring confidential information processing.
    9. Assists in the planning and development of an annual budget; analyzing variances; initiating corrective actions.
    10. Develops and leads a high performance team by participating in recruiting, selecting, orienting, and training of Post-Acute Care Directors.
    11. Works in concert with Physicians, Nursing, Case Management, Directors, Providers and community social support programs facilitating care of patients in the post-acute care setting.
    12. Communicates with the on-site Case Management staff to ensure seamless transitions of care for our patients.
    13. Assists in the development of a quality improvement processes as it relates to the care of patients in the post-acute setting including monitoring key elements such as Admission volume, Cost, LOS, Readmission, and ED Admissions.
    14. Coordinates and oversees multi-disciplinary team care meetings to assure appropriate throughput.
    15. Designs and coordinates formal and informal program training sessions for internal and external constituents related to billing, regulatory requirements and quality.
    16. Assesses, reports, and communicates program status on a periodic basis to all program stakeholders.
    17. Develops and uses the LEAN system to continuously evaluate and address internal and external opportunities for improvement.
    18. Contributes to team effort by accomplishing related results as needed

     

    Experience

    • 5 years post-acute care operations management required
    • 3-5 years inpatient case management preferred

    Education

    • Bachelor’s degree in nursing, social work, healthcare/business required.
    • License in a Health Related Field desirable

    Special Knowledge, Skills, Abilities

    The ability to lead and manage complex projects that are strategic in nature and scope; Experience/knowledge of moving patients from acute to post-acute settings; Current clinical knowledge and experience, particularly with the senior and unfunded populations; the ability to collaborate and motivate other healthcare providers; Effective problem solving skills; Experience with performance  measurement, outcome analysis, monitoring tools; computer skills; Excellent oral and written skills; excellent organizational skills with demonstrated ability to complete projects on time and within budget; Pro-active/effective team leadership skills.
