    1. Job responsibilities include those listed in the competency document.
    2. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
    3. The nutrition counselor will provide state-of-the-art nutrition education in a variety of settings to patients and their families/care partners for both inpatient and outpatient settings.
    4. The nutrition counselor will lead and develop nutrition education for diabetes self-management training and medical nutrition therapy services in the outpatient setting
    5. Participates in the development and delivery of diabetes education for both inpatient and outpatient settings.
    6. Participates in monthly diabetes support group meetings
    7. Serves as a resource for quality diabetes nutrition information for patients and other health care team members.
    8. Perform patient care tasks according to hospital- and physician-approved protocols.
    9. Adequately and accurately document patient interactions and progress in patients’ medical record.  Follow hospital and program requirements 
    10. Communicates with referring and treating physician on patient care
    11. Participate in all care conference meetings
    12. Direct and schedule the patient to other modalities as the treating physician requires
    13. Present nutrition informational programs for community, professionals and potential clients as requested.
    14. Promote Center activities and classes via media promotions as needed.
    15. Provides evening and/or weekend coverage as needed for program activities
    16. Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery.
    17. Schedules initial and follow up appointments for nutrition and diabetes patients.
    18. Bill for services provided when applicable following all hospital and programmatic guidelines
    19. Serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, community resources and payers to reduce redundancy and fragmentation of care, ensures that the necessary services are provided at the most appropriate level of care and that there is a smooth progression of the patient through the health care system
    20. Provides language specific education, psychosocial support, and necessary resources for newly diagnosed patients pertinent to their specific disease, and their families (language specific), and coordinates appropriate referrals for those needed services.
    21. Utilizes appropriate health and safety equipment to protect both patient and staff.
    22. Maintains knowledge and skills consistent with current practice guidelines. Provides self with continuing education through participation in educational programs, workshops, and research programs, which will lead to improvement of patient care and self-development.
    23. Demonstrates competency in, but not limited to, the following in the Chronic Care Management setting (refer to McLeod Health Competency Checklist):
    • Age specific training
    • Abuse, neglect, and Exploitation
    • Environment of Care/Emergency Management
    • Infection Control Precautions
    • Care of he Deaf and Hard of Hearing
    • Nutriion Assessment
    • Nutrition Counseling
    • Development of Individualized Meal Plans
    • Diabetes Monitoring Tools

     

    QUALIFICATIONS /TRAINING:

     

    • Certified Diabetes Educator and/or 3 years of experience with diabetes

     

     LICENSES/CERTIFICATIONS/REGISTRATIONS/EDUCATION:

     

    • Required College degree in Dietetics
    • Required Certification: Registered Dietitian with the Commission on Dietetic Registration.
    • Obtain personal NPI number within 3 months of hire
