  • Responsibilities

    CERTIFIED DIABETES EDUCATOR

    • Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
    • The Diabetes Educator will provide state-of-the-art diabetes education in our office and the Children’s Hospital.
    • Lead and develop education for diabetes self-management training in the outpatient setting.
    • Participates in the development and delivery of diabetes education for both inpatient and outpatient settings.
    • Serves as a resource for quality diabetes information for patients and other health care team members.
    • Adequately and accurately document patient interactions and progress in patients’ medical record.
    • Communicates with referring and treating physician on patient care.
    • Participate in all care conference meetings.
    • Present Diabetes related programs for community, professionals and potential clients as requested.
    • Promote Center activities and classes via media promotions as needed.
    • Provides evening and/or weekend coverage as needed for program activities
    • Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery.
    • Schedules initial and follow up appointments for diabetes patients.
    • Serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, community resources and payers to reduce redundancy and fragmentation of care, ensures that the necessary services are provided at the most appropriate level of care and that there is a smooth progression of the patient through the health care system
    • Provides language specific education, psychosocial support, and necessary resources for newly diagnosed patients pertinent to their specific disease, and their families (language specific), and coordinates appropriate referrals for those needed services.
    • Utilizes appropriate health and safety equipment to protect both patient and staff.
    • Maintains knowledge and skills consistent with current practice guidelines. Provides self with continuing education through participation in educational programs, workshops, and research programs, which will lead to improvement of patient care and self-development.
    • Demonstrates competency in, but not limited to, the following in the Chronic Care Management (refer to McLeod Health Competency Checklist):
      • Age Specific Education Training
        • Abuse, Neglect, and Exploitation
        • Environment of Care/Emergency Management
        • Infection Control Measures
        • Care of Deaf and Hard of Hearing Patient
        • Safety Initiatives
        • Diabetes Monitoring tools
        • Patient Education/Counseling
    • Other duties as assigned.

    WORK SCHEDULE: 80 hours biweekly as scheduled 

    QUALIFICATIONS /TRAINING: 

    • Certified Diabetes Educator and/or 2 years of experience with diabetes

      

    LICENSES/CERTIFICATIONS/REGISTRATIONS/EDUCATION: 

    • CDE with valid RN or RD license or certification
