    Who You Are

    You are a dedicated and empathetic Nurse Practitioner with at least two years of experience, preferably with a background in nephrology or experience in mobile patient visits, although this is not a requirement. Your qualifications include:

    • Master’s or Doctorate level degree in Nursing.

    • Current board certification and state-specific licensure.

    • Proficiency in clinical assessment, diagnosis, and patient care.

    • Strong communication skills and the ability to work collaboratively within an interdisciplinary team.

    • Experience with electronic medical records and basic computer skills.

    • Physical ability to perform home visits and navigate a variety of patient environments.

    • A commitment to providing exceptional patient care, building trust, and fostering strong patient relationships.

    What You Will Do

    As a vital member of our interdisciplinary clinical team, you will:

    • Deliver advanced nursing care for CKD and ESKD patients across various settings, including patients' homes, physician offices, and through telehealth.

    • Work closely with social workers, care coordinators, pharmacists, dietitians, and physicians to manage patient care effectively.

    • Be responsible for patient assessments, care planning, diagnostics, and follow-ups.

    • Utilize your knowledge of value-based care to improve patient outcomes.

    • Participate in clinical rounds, interdisciplinary team meetings, and provide clinical support.

    • Serve as a liaison among patients, families, and healthcare providers to ensure coordinated and consistent care.
