    Who You Are

    • A board-certified physician in Internal Medicine, Family Medicine, or Geriatrics, with at least three years of experience in primary care, especially caring for the elderly with complex chronic conditions.
    • You excel in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of complex patients.
    • An exceptional communicator and team player, focused on results and proficient in managing to clinical, financial, and patient satisfaction outcomes.
    • Experienced and supportive of Value-Based Care models and collaborative practice settings.
    • Ready to travel to member locations, capable of delivering in-person and virtual care, and willing to participate in on-call rotations.
    • You hold current state-specific licensure, a DEA license, BLS certification, and comply with the company's driving standards.

    What You'll Do

    • Provide top-tier primary care for frail elderly patients with chronic illnesses, in close collaboration with a medical director and an interdisciplinary team.
    • Take full accountability for patient care and outcomes, maintaining a proactive approach in prevention, care management, and minimizing complications.
    • Act as a key advocate and navigator for patients, ensuring their care needs are met and aligned with their life goals.
    • Stay abreast of the latest in care strategies, incorporating evidence-based medicine and best practices into patient care.
    • Deliver clinical care autonomously across various settings, including patient homes, skilled nursing facilities, and through telehealth, ensuring a consistent and high-quality patient experience.
