    Join our mission-driven team and make a difference in the lives of frail older adults. We are seeking a passionate Registered Nurse to provide high-quality, integrated care as part of our PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team.

    In this role, you will be responsible for managing and delivering direct nursing care to PACE participants in a variety of settings, including clinics, care centers, homes, virtual visits, skilled nursing facilities, and more. As a critical member of the interdisciplinary team, you will collaborate closely with other providers to complete comprehensive assessments, develop personalized care plans, and ensure timely service delivery.

    Key Responsibilities:

    • Serve as the point person for the interdisciplinary team, ensuring progress on participants' care plans
    • Deliver nursing care in various settings, including primary care, medication administration, wound care, oxygen therapy, and more
    • Conduct comprehensive nursing assessments of physical, psychosocial, and behavioral status
    • Identify and independently initiate emergency interventions as needed
    • Educate participants, caregivers, and staff on personalized care plans and response strategies
    • Monitor and evaluate therapeutic interventions, recommending changes to improve health and well-being
    • Provide on-call coverage as needed

    Ideal Candidate:

    • Passion for serving high-risk seniors and frail older adults
    • Minimum 2 years of experience caring for adults, chronically disabled or frail, or elderly populations
    • Ability to function independently in the day-to-day management of clinical needs and program operations
    • Strong active listening and conflict management skills
    • Adaptability to multiple clinical settings
    • Training in wound care, IVs, and blood draws
    • Active, unrestricted RN license in California
    • BSN preferred
    • CPR and First Aid certification
    • Alzheimer's certification
    • Proficiency in English and Spanish (preferred)
