    Unique Opportunity for a Mission-Driven Registered Nurse in Los Angeles, CA

    Our client is a trailblazing health organization dedicated to transforming the care and well-being of vulnerable older adults and frail seniors through their innovative PACE (Program of All-Inclusive Care for the Elderly) model. Founded in 2021 by mission-aligned healthcare leaders, they specialize in providing comprehensive care and coverage, enabling older adults to thrive in their homes and communities. They are now looking for a compassionate and dedicated Registered Nurse to join their team as an Enrollment Registered Nurse, a pivotal role in expanding their PACE program to underserved communities in Los Angeles.

    Who You Are:

    • You hold an active Registered Nurse License in California, with an Associate's degree from an accredited nursing school; a BSN is preferred.
    • Passionate about delivering exceptional care to frail elders and disabled older adults, with 3-5 years of healthcare experience, including working with the elderly.
    • You are organized, possess excellent communication skills, and are proficient in documentation for CRM/EHR systems.
    • Bilingual in English and Spanish is preferred, along with current CPR certification or the ability to obtain it within 30 days of hire.
    • You have a valid CA driver's license, personal transportation, a good driving record, and auto insurance as required by state law.

    What You'll Do:

    • Collaborate with the Outreach Specialist and Interdisciplinary Team (IDT) to ensure successful enrollment of participants in the PACE program.
    • Conduct home visits to evaluate prospective participants for potential eligibility and perform home safety evaluations.
    • Participate in Enrollment Eligibility meetings, complete Nursing Facility Level of Care assessments, and maintain accurate documentation in CRM software.
    • Support the Center Director in developing participant retention strategies and maintain expertise in eligibility, enrollment, and program specifications for the PACE Program.
    • This hybrid role, based in Los Angeles, requires remote fieldwork and onsite presence at the Los Angeles facility up to 5 days per month.
