Job Title: Medical Assistant – In-Clinic & In-Home

Position Summary:
As a Medical Assistant at Care Connectors, you will provide hands-on clinical support in both our clinics and patients’ homes, focusing on Annual Wellness Visits (AWVs), chronic care support, and preventive screenings such as Diabetic Retinopathy Exams (DRE) and Osteoporosis Management for Women (OMW). You will be part of a mission-driven team helping patients stay healthy, independent, and connected to the care and resources they need.
What You’ll Do
· Perform patient intake in-clinic and in-home: vitals, history, medication reconciliation, and social determinants of health.
· Support providers with AWVs and chronic disease visits, ensuring all required elements and quality measures are completed.
· Complete Diabetic Retinopathy Exams (DRE) using portable retinal imaging equipment; educate patients on the purpose of eye screening in diabetes and coordinate referrals when needed.
· Assist with Osteoporosis Management for Women (OMW) by identifying eligible patients, performing or coordinating bone density/osteoporosis screenings, and reinforcing follow-up plans.
· Document all encounters accurately in the EHR and support correct coding and quality capture.
· Manage outreach and scheduling for in-home visits, including confirming appointments, routing, and directions.
· Safely transport, set up, and maintain mobile medical equipment (e.g., DRE camera, diagnostic devices, vital sign monitors).
· Reinforce care plans, provide patient education, and connect patients to community and social resources when appropriate.
· Follow all HIPAA, infection control, and safety protocols across clinic and home environments.
What We’re Looking For
· Completion of a Medical Assistant program; MA certification (CMA, RMA, or equivalent) preferred.
· 1+ year experience in primary care, geriatrics, or mobile/in-home care is a plus.
· Comfortable working independently in the field and as part of a clinic-based team.
· Valid driver’s license, reliable transportation, and ability to travel locally for in-home visits.
· Strong communication skills, customer-service mindset, and ability to build rapport with seniors and complex patients.
· Basic proficiency with EHRs and mobile technology (laptop/tablet, secure apps).
