House of Hope, a well‑established nonprofit providing residential treatment for individuals with substance use disorders, is seeking a Medical Care Coordinator. This role is ideal for someone who is highly organized, compassionate, and committed to supporting clients through their recovery journey.
As a Medical Care Coordinator, you will play a crucial role in ensuring individuals receive necessary medical services, attend appointments, and stay consistent with prescribed medications.
Key Responsibilities
Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy.
Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources.
Ensure timely medication distribution and support medication compliance.
Conduct daily medication checks and document compliance in the EMR system.
Monitor new or updated medications to ensure appropriate documentation and continuity.
Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies.
Assist counselors with discharge planning.
Perform additional duties as assigned.
Required Qualifications
High School Diploma required; Bachelor’s Degree preferred.
1 year of Care Coordinator experience.
2 years of Behavioral Health experience.
Experience working with individuals with substance use disorders preferred.
Strong communication and problem‑solving skills.
Ability to work effectively with a diverse population.
Computer literacy and comfort with EMR systems.
