    House of Hope is a well-established nonprofit dedicated to providing residential treatment for individuals overcoming substance use disorders. We are seeking a highly organized, compassionate, and dedicated Medical Care Coordinator to join our team.

    In this vital role, you will bridge the gap between our clients and the essential medical services they need. By managing appointments, securing benefits, and ensuring medication compliance, you will provide the stability and support our clients need to successfully navigate their recovery journey.

    Key Responsibilities

    Coordinate Care: Assist clients in accessing critical medical, dental, vision, housing, financial, and legal advocacy services.

    Navigate Benefits: Support clients in obtaining, tracking, or reactivating vital resources like Medicaid and disability benefits.

    Manage Medications: Oversee timely medication distribution, conduct daily compliance checks, and log updates accurately in the EMR system.

    Ensure Continuity: Monitor new prescriptions to guarantee seamless documentation and uninterrupted client care.

    Handle Documentation: Complete and upload all incidental funds paperwork with strict accuracy and adherence to agency policy.

    Support Transitions: Collaborate closely with counselors to build thorough, safe, and effective discharge plans.

    Required Qualifications

    Education: High School Diploma required; Bachelor’s degree preferred.

    Experience: 1+ years of Care Coordination and 2+ years in Behavioral Health.

    Specialization: Experience working with individuals navigating substance use disorders is highly preferred.

    Tech Skills: Strong computer literacy with hands-on comfort using Electronic Medical Record (EMR) systems.

    Core Traits: Exceptional communication, proactive problem-solving, and a deep ability to work effectively with diverse populations.
