Job Summary
We are looking for a qualified Medical Case Manager to join our team! You will play a crucial role in evaluating our consumers medical documentation in collaboration with caregivers, family members, and medical professionals, etc.

Responsibilities
Implement standards of care for medical social work services
Participate in consumers’ plans of care.
Perform internal audits and review of all medial related documentation
Assess the psychosocial status of the patients as related to their illness
Review follow-up visits to assess and continue the plans of care
Plan interventions with team members based on consumers' needs and findings
Maintain accurate and up-to-date records
Qualifications
Minimum one year of experience in health care and social work
First Aid/CPR certification
Valid driver’s license
Experience in Human Service, Case Management and Medical industries is a plus!
Experience working independently and managing systems
QMAP Certified
