Position Overview:
T12 Technologies is seeking a Full-Time Medical Director who will provide clinical leadership and oversee the delivery of quality medical care to Texas Veterans Nursing Home residents. This position ensures compliance with federal and state healthcare regulations, promotes best practices in geriatric and long-term care, and collaborates with interdisciplinary teams to enhance our veteran residents' health outcomes and quality of life.
Key Responsibilities
· Resident-Centered Clinical Care: Conducted or oversaw comprehensive medical assessments and collaborated with the interdisciplinary team to maintain and regularly update each resident’s care plan. Provide direct patient care, including acute and chronic disease management, preventive care, and coordination of specialty services. Facilitate effective communication with residents and their families regarding medical status, treatment options, and prognosis.
· Medical Oversight & Staff Supervision: Supervise attending physicians, nurse practitioners, and physician assistants to ensure clinical competence, appropriate documentation, and high-quality care. Facilitate medical staff orientation, training, and continuing education on topics such as geriatric care, dementia, pain management, and trauma-informed care. Lead case conferences and clinical reviews; participate in care planning meetings and interdisciplinary team discussions.
· Regulatory Compliance & Quality Assurance: Ensure compliance with all applicable regulations from CMS, Texas Health and Human Services, and the Department of Veterans Affairs. Prepare for and participate in health department and accrediting agency surveys; assist with developing corrective action plans for deficiencies. Implement and oversee infection control protocols, fall prevention strategies, and adverse event tracking. Contribute to the facility’s Quality Assurance and Performance Improvement (QAPI) program.
· Veteran-Centered Leadership & Collaboration: Champion a care philosophy that honors the service and unique needs of the veteran population, including those with PTSD, traumatic brain injury (TBI), and other military-related health conditions.
Act as a liaison to local VA facilities, community providers, and hospitals to ensure care continuity and access to appropriate services. To support seamless care transitions, provide medical input on admissions, transfers, and discharge planning.
Qualifications
· MD or DO degree from an accredited medical school.
· Current and unrestricted license to practice medicine in the state of Texas.
· Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine.
· Minimum of five (5) years of clinical experience in the long-term health care field, preferably in a skilled nursing facility or veterans' care setting.
· Demonstrated leadership experience in a medical director, chief of staff, or similar clinical leadership role.
· Working knowledge of long-term care regulations, including CMS standards, Texas licensing requirements, and VA policies.
Preferred Qualifications:
· Experience caring for military veteran populations.
· Certification as a Certified Medical Director (CMD) by AMDA or a similar credential.
· Experience with infection prevention, medication stewardship, and palliative care models.
