Primary Care Physician
Position Summary:

We are currently recruiting a provider to join our post-acute care practice, which serves residents in assisted living, skilled nursing, and rehabilitation facilities. Key Responsibilities:
- Act as the attending physician for patients in assigned post-acute care settings.
- Provide medical oversight and leadership as the facility’s Medical Director.
- Deliver high-quality, evidence-based, patient-centered care.
- Collaborate with and support a dedicated team of nurse practitioners, typically one per facility.
- Offer medical insight and guidance to enhance the quality of care and patient outcomes.
- Conduct clinical rounds and maintain documentation in the EMR system.
- Serve as a resource for interdisciplinary teams to improve the overall patient experience.
Schedule & Work Environment:
- Flexible scheduling — design your ideal workweek: choose anywhere from 1 to 5 days per week based on your workload preference.
- Monday through Friday, daytime hours only — no evenings or weekend shifts.
- Typical frequency of 1–2 facility visits per week per site, based on acuity and census.
- Minimal call obligations — phone call only, shared rotation. In some cases, no call depending on NP coverage.
- Control your own hours and daily itinerary.
- All major holidays off.
Qualifications:
- MD or DO with an active and unrestricted medical license.
- Previous experience in a hospital, post-acute, or long-term care setting preferred.
- Proficiency in electronic medical records (EMR); familiarity with Dragon dictation is a plus.
- Strong interpersonal and leadership skills.
- Minimum of 1 year of clinical experience as a physician.
- A strong interest in independent, flexible practice models.
Compensation & Benefits:
- Total compensation range: $200,000 to $450,000+, depending on your preferred workload.
- Base salary with uncapped monthly bonus potential.
