General Summary
The Medical Records Clerk demonstrates the CACOST organizational standards of accountability, teamwork, integrity, quality service, and excellence in their everyday duties as a member of the healthcare team. This role is responsible for managing incoming and outgoing clinical documentation, coordinating records requests, supporting compliance with HIPAA and federal transparency requirements, and ensuring accurate and timely handling of protected health information (PHI) in accordance with Health Center protocols and applicable laws.
Primary Responsibilities
Processes incoming and outgoing patient forms and documents, ensuring accurate uploading and attaching to the correct patient chart, and assigning to the appropriate staff member for follow-up.
Assist with coordination of home health and DME forms to include, patient communication notes, certificates of medical necessity, certifications and re-certifications.
Process medical record requests in compliance with applicable state and federal requirements.
Calculates, collects, and documents appropriate medical record release fees.
Assists with medical record audit requests by providing requested documents within required timeframes.
Prepares and compiles information for Good Faith Estimates, ensuring timely and accurate estimates in alignment with federal transparency and billing guidelines.
Maintains patient confidentiality in accordance with all applicable federal and state laws and regulations.
Maintains medical equipment in accordance with training and manufacturers’ instructions.
Utilizes facilities and agency property according to CACOST policy.
Communicates clearly and professionally with patients and authorized representatives in accordance with clinic policies.
Interacts with all co-workers, vendors, and third parties in a professional manner.
Follows the proper chain of command.
Adheres to agency program requirements, policies and procedures.
Performs other duties as assigned by supervisor.
Education/Certifications/Licensure
Required
High School diploma or equivalent.
Must have a valid driver's license and a safe driving record
Must pass a pre-employment physical
Must pass a criminal history background check.
Skills
Required
Experience with computer systems.
Strong written and verbal communication skills.
Ability to work collaboratively in a team environment.
Preferred
Knowledge of medical terminology
Experience with electronic medical record platforms
Physical Requirements
If a preliminary offer of employment is extended, a physical will be required before the offer can be accepted.
Light to medium physical duties include standing, walking, reaching, bending, talking for up to eight (8) hours a day. Must have good vision. Must be able to lift thirty-five pounds. Lifting will be primarily confined to occasional on premises lifting of boxes of office supplies and minor office equipment. Non-physical demands include performing multiple tasks simultaneously, the ability to meet multiple deadlines, judgement decisions and working closely with others as part of a team.
