Valley Home Health Care is a Medicare-certified home health agency committed to delivering high-quality, patient-centered care throughout Southern Nevada. We are seeking a detail-oriented Medical Records Clerk to join our growing administrative team.
Position Summary
The Medical Records Clerk is responsible for managing and maintaining patient medical records, ensuring documentation is complete, accurate, organized, and compliant with Medicare, ACHC, HIPAA, and agency policies. This role works closely with Intake, Clinical Managers, QA, and field clinicians to support timely processing of patient records throughout the episode of care.
Responsibilities
Review incoming patient documentation for completeness.
Upload, organize, and maintain electronic medical records (EMR).
Process physician orders, referrals, hospital records, laboratory results, and other clinical documents.
Monitor outstanding physician-signed orders and follow up with provider offices.
Ensure required documentation is received prior to admission and throughout the patient's episode.
Maintain accurate patient charts in accordance with Medicare and ACHC standards.
Coordinate with Intake, QA, Nursing, Therapy, and Medical Records departments.
Track missing documentation and communicate deficiencies to appropriate staff.
Prepare records for audits, surveys, and quality reviews.
Process discharge and archived records according to agency policy.
Maintain HIPAA compliance and patient confidentiality at all times.
Perform other administrative duties assigned by management.
Qualifications
Required
Minimum 1 year experience in medical records, healthcare administration, or medical office.
Excellent organizational skills and attention to detail.
Strong computer skills.
Experience with Microsoft Office (Word, Excel, Outlook).
Excellent written and verbal communication skills.
Ability to prioritize multiple tasks in a fast-paced environment.
Strong understanding of HIPAA confidentiality requirements.
Preferred
Experience in Home Health (strongly preferred).
Experience with Medicare documentation requirements.
Familiarity with home health EMR systems (Finale Health experience is a plus).
Knowledge of physician orders, Plan of Care (485), OASIS documentation, and home health workflows.
