Job Description:

As a Medical Social Worker at Roadrunner Home Healthcare, you will play a vital role in assisting patients and their families in navigating the emotional, social, and financial challenges of illness, aging, and recovery at home.

Key Responsibilities:

Assess patients’ psychosocial status and support needs

Provide emotional counseling and crisis intervention

Assist families with long-term planning and healthcare decisions

Connect patients to community resources, financial aid, or in-home support

Collaborate with nurses, therapists, and physicians to coordinate care plans

Educate patients and families on coping strategies, advanced directives, and more

Maintain accurate documentation in accordance with Medicare, Medicaid, and agency policies

Qualifications:

Master’s Degree in Social Work (MSW) from an accredited institution

Licensed in the state of New Mexico as an LCSW, LMSW, or equivalent

Minimum 1 year of experience preferred

Strong interpersonal, organizational, and communication skills

Reliable transportation and valid driver’s license
