    Medical Social Worker – Home Health (Corvallis, OR)

    We are seeking a dedicated and compassionate Medical Social Worker to join our home health team serving patients in Corvallis, Oregon and the surrounding areas. In this vital role, you will conduct comprehensive assessments, develop tailored intervention plans, and provide resources and support to patients, families, and caregivers in need.

    Key Responsibilities

    • Conduct psychosocial assessments to understand patients’ needs within the context of their illness and environment.

    • Develop and implement personalized intervention plans based on assessment findings.

    • Maintain accurate clinical records for all referred patients.

    • Provide information and referral services to patients, families, and caregivers.

    • Serve as a liaison to community agencies, ensuring patients and their families have access to the support they need.

    Qualifications

    • Graduation from a Master’s program in social work accredited by the Council on Social Work Education.

    • Active clinical social worker license issued by the Board of Behavioral Sciences.

    • For Oregon or Washington residents, an LCSW or AWS license is required.

    • At least one year of healthcare experience (home health care experience preferred).

    • Strong verbal and written communication skills, with exceptional organizational abilities.

    • Current BLS and hands-on CPR certification.

    • Valid driver’s license and reliable, insured transportation.
