Key Responsibilities:

• Implement and manage patient care plans, offering assessments and patient reports.
• Support and educate family members on patient self-care.
• Coordinate home health services, collaborating with an interdisciplinary team to ensure comprehensive care.

What We’re Looking For:

• Licensed Medical Social Worker in good standing with the state of Arizona.
• Master’s Degree in Social Work.
• At least one year of supervised post-certification experience in a healthcare setting (home health experience preferred).
• Successful completion of a background check and drug screen.
