Medical Social Worker – Full-Time 📍 Luna Del Valle Hospice – Compassionate End-of-Life Care

Position Overview: We are seeking a Full-Time Medical Social Worker (MSW or BSW under MSW supervision) to join our interdisciplinary hospice team. In this role, you will provide vital emotional, psychosocial, and resource-based support to patients and families navigating the end-of-life journey. This is a field-based role that requires travel to patient homes and facilities, along with close collaboration with nurses, chaplains, physicians, and caregivers.

Key Responsibilities:

Perform comprehensive psychosocial assessments for patients and families

Provide counseling, crisis support, and guidance to assist with emotional, psychological, and spiritual concerns

Assist families in coping with grief, loss, and caregiving stress

Collaborate with the interdisciplinary team to develop and implement individualized care plans

Educate patients and families on available resources and connect them to appropriate community-based services

Understand and help ensure appropriate insurance coverage for services, including Medicare and Medicaid hospice benefits

Participate in discharge planning and care transitions as needed

Maintain timely, accurate documentation in accordance with state and federal regulations and company policy

Ensure patient dignity, rights, and confidentiality are maintained at all times

Travel to patient homes and facilities throughout the service area

Participate in occasional on-call rotation (if applicable)

Qualifications:

Required:

MSW from a CSWE-accredited program

OR BSW from a CSWE-accredited program with supervision by an MSW (per CMS hospice requirements)

Current and valid Social Work license

Valid driver’s license and proof of auto liability insurance

Reliable transportation for in-field patient visits

Preferred:

At least 1 year of experience in healthcare, hospice, or palliative care setting

CPR certification

Familiarity with Medicare/Medicaid hospice guidelines

Strong communication and interpersonal skills

Ability to work independently and manage emotionally sensitive situations with compassion

Schedule:

Full-Time

Day shift

Flexibility required to meet patient needs

Occasional on-call availability may be required
