Essential Functions:
- Conduct comprehensive psychosocial assessments to identify barriers related to mental health, social determinants of health (SDOH), housing, food access, transportation, financial strain, and caregiver support.
- Provide brief counseling, crisis intervention, and motivational interviewing to support patient engagement in care plans.
- Collaborate with Rendr360 nurse care managers and providers to coordinate care for high risk‑ patients.
- Develop individualized care plans addressing psychosocial, behavioral, and community resource needs.
- Facilitate referrals to behavioral health services, community agencies, and government programs (e.g., Medicaid/Medicare benefits, home care, housing support).
- Document all encounters in the electronic medical record according to care management and compliance standards.
- Support transitional care, chronic care management, and disease management programs by addressing psychosocial risks contributing to hospital utilization.
- Participate in interdisciplinary meetings, case discussions, and quality improvement activities.
- Advocate for patients and families to ensure access to appropriate care and services.
Qualifications:
- Master’s Degree in Social Work (MSW), required.
- Licensed Masters of Social Work (LMSW), preferred.
- 1 year of medical or healthcare social work experience (hospital, primary care, home care, or care management settings), preferred.
- Trilingual proficiency in Cantonese, Mandarin, and English required.
- Experience working with diverse, immigrant, and older adult populations strongly preferred.
- Strong communication, problem solving, and care‑ coordination skills.
- Ability to work collaboratively in a multidisciplinary team environment.
