    We provide highly personalized care for each patient. Patients have the same care partner – a Certified Medical Assistant (CMA) – that calls and stays in touch with the patient each month. The patients and care partners know each other well, building strong relationships and trust. Care partners know their patient's health and health history well, so they can provide comprehensive, continuous support to their patients. Patients and practices each see the Medsien care team as an extension of the practice. Strong relationships between patients and the care team build engagement, which improves patient health outcomes. Personalized care is a key component of Medsien programs' long-term success.

    Care Partners at Medsien enroll, support and manage the care management component of your care management eligible patients. Each patient has a dedicated Care Partner, so every month (and more often if needed), the same Care Partner will contact your patient to check in on how they are doing, monitor their health status, and conduct all aspects of a CCM, RPM, etc. appointment. The Care Partner is the patient's go to for questions, care management and serves as the patient's liaison to the practice. Strong relationships are built over time, strengthening compliance and the quality of care patients receive, their health outcomes and patient satisfaction.

    Are you a CMA or RMA looking for a work from home position? If so, this may be the perfect opportunity for you!

    In this role you will assist providers and their health care teams in clinics and offices by performing a variety of patient-related tasks part of the Chronic Care Management program.

    You will get the chance to work in an innovative and fast-paced environment that puts patients first. All while working fully remote, having a flexible schedule and helping patients across the country.

    Responsibilities

    • Triaging patient calls

    • Complete patient surveys

    • Confirm medication compliance with patients

    • Reconcile EHR data and updated consult notes

    • Complete intervention recommendations from Provider

    Qualifications

    • CMA or RMA License(Nationally recognized)

    • Knowledge of third-party payer requirements

    • Familiarity with electronic medical records

    • Strong aptitude for detail, and ability to work independently

    • Strong organizational, problem solving, communication, and interpersonal skills
