  • Responsibilities

    Job Description

    Provides evening and weekend social work and case management services to patients on the inpatient Geriatric Med-Psych unit, based on local and national Professional Social Work Standards and Code of Ethics.  Completes psychosocial assessment on assigned inpatients within 72 hours of admission. Assesses discharge planning needs for assigned patients and implements completed aftercare plan.  Maintains contact with patient’s family, other mental health providers, and other collaterals in order to facilitate their input and involvement in treatment and discharge planning.  Conducts utilization review as requested or required on all assigned patients when patient insurance plan requires. Facilitates group treatment in the milieu.   

  • Qualifications

    Qualifications

    • Master’s Degree from an accredited School of Social Work. 
    • LCSW required, LICSW preferred.
    • Previous experience in inpatient psychiatry or mental health setting working with major mental illness and substance abuse diagnoses preferred.
    • Ability to communicate effectively verbally and in writing.  Ability to work as a team player.
    • Ability to engage a wide variety of patients and collaterals in the acute treatment and discharge planning process.
    • Ability to identify and access community resources.
