Position Summary: We are seeking a compassionate and experienced individual to join our TMS (Transition Management Services) team. This team member will play a key role in providing person-centered services and housing support to adults with SPMI. This position involves care coordination, skill-building, service planning, and advocacy to help individuals successfully transition to and sustain community-based living.
Key Responsibilities:
Provide direct support to adults with SPMI in identifying and obtaining permanent supportive housing
Develop housing resources in connections in the community in order to assist your clients in finding permanent supportive housing
Assist individuals with developing daily living skills, self-advocacy, and community integration
Monitor client progress, maintain accurate documentation, and ensure compliance with all regulations
Promote recovery, independence, and housing stability through person-centered approaches
Participate in interdisciplinary team meetings, trainings, and supervision
Qualifications:
North Carolina criteria for Qualified Mental Health Professional (QP) OR Associate Professional (AP) OR Licensed Occupational Therapist.
QP - Bachelor’s degree in a human services field with at least 2 years of full-time adults MH experience, or Master’s degree in a human services field with at least 1 year of experience
AP - Bachelor's Degree without 2 - 4 years of experience
Licensed Occupational Therapist - Master's level licensed clinician
Experience working with adults with serious mental illness in community-based settings preferred
Knowledge of housing resources, case management, and recovery-oriented care
Strong communication, documentation, and organizational skills
Valid NC driver’s license and reliable transportation
Ability to work independently while collaborating within a team environment
Schedule:
8 hour shift
Day shift
Monday to Friday
Weekends as needed
On-Call rotation
