    Position Summary

    We are seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to provide high-quality mental health counseling and case management services to adults, including people living with HIV (PLWH), experiencing mental illness and/or substance use disorders.

    This role is part of a multidisciplinary care team focused on delivering integrated, client-centered services. The clinician will address a full spectrum of needs including crisis intervention, behavioral health treatment, substance use services, medical coordination, and support with housing, benefits, and community resources.

    Essential Duties & Responsibilities

    Conduct outreach and engagement activities using a non-coercive, client-centered approach

    Perform comprehensive mental health assessments, including mental status exams and clinical formulations

    Complete SBIRT screenings and collaborate with clients to develop individualized service plans

    Provide individual counseling, crisis intervention, and case management services

    Facilitate psychoeducational and therapeutic groups using approved curricula

    Collaborate with interdisciplinary team members to coordinate and implement holistic treatment plans

    Assist clients in accessing medical care, substance use treatment, benefits, and stable housing

    Escort clients to appointments and community-based services as needed

    Monitor and support client health, safety, and overall well-being

    Complete incident reports and participate in debriefings as required

    Maintain accurate, timely documentation in the electronic medical record (EMR) system in compliance with program and funding requirements

    Participate in clinical meetings, case conferences, and required trainings

    Ensure compliance with all confidentiality and privacy regulations, including HIPAA and 42 CFR Part 2

    Required Qualifications

    Active LCSW or LPC license

    Master’s degree in Social Work, Counseling, or a related field

    Valid driver’s license

    Computer proficiency, including Microsoft Office and electronic medical records

    Preferred Qualifications:

    Certified Alcohol & Drug Counselor (CADC) certification

    Experience working with co-occurring mental health and substance use disorders

    Experience working with individuals living with HIV (PLWH)

    Demonstrated leadership experience (formal or informal)

    Skills & Competencies

    Strong clinical assessment and diagnostic skills, including risk assessment for harm to self or others

    Proficiency in evidence-based practices and treatment modalities

    Knowledge of trauma-informed care, recovery-oriented models, and Assertive Community Treatment (ACT)

    Ability to develop, implement, and monitor individualized treatment plans

    Crisis intervention and de-escalation skills (or willingness to be trained)

    Excellent communication and interpersonal skills

    Strong organizational skills with attention to detail and documentation accuracy

    Ability to manage competing priorities in a fast-paced environment

    Cultural competence and ability to work effectively with diverse populations

    Ability to work independently while collaborating within a team environment

    Creative problem-solving and adaptability

    Additional Requirements

    Ability to gather comprehensive client histories, including mental health, substance use, medical, and social factors

    Ability to assess family, social, and occupational functioning

    Commitment to providing services within scope of practice and ethical guidelines

    Ability to interact respectfully and effectively with clients, colleagues, and community partners including medical providers and law enforcement
