    Job Description

    The Ideal candidate for this position has at least a Bachelor's degree in psychology, social work, sociology or recreational therapy will manage the social service and activity portions of the plan of care for each resident on the Boulevard.

     Job Duties Include:

    ·         Advocate for resident’s rights in all interactions with residents, families and staff.

    ·         Key member of the care plan team in developing and maintaining the social and activity needs of each resident.

    ·         Complete all MDS assessments for the Social Service and Activity sections.

    ·         Develop, schedule and provide appropriate engagement centers and individualized level of activity for all resident on the Boulevard.

    ·         Facilitate all care plan meetings with families as scheduled per MDS.

    ·         Document, manage and report all behaviors per clinical policy and work directly with facility appointed psychologist and/or psychiatrist on medication management.

    ·         Facilitate monthly meetings with Boulevard Clinical Unit Nurse, DON, SSD and Administrator on plan of care.

    ·         Provide dementia training and certification to all new team members.

    ·         Assure that the Boulevard unit is properly maintained and engagement centers, common living area and dining room are easily accessible to all residents.

  • Qualifications

    Qualifications

    • Bachelor's degree in psychology, social work, sociology or recreational therapy

    • Experience working with patients or residents in a memory care / dementia unit

    • Experience in a long term care facility is preferred
