    Mindoula's teams of case and care extenders, supported by technology including a mobile engagement app, proprietary psychometrics, and predictive analytics, are the key to its success. Mindoula is expanding rapidly across multiple business lines in multiple states and is seeking an experienced Care Extender for the Suicide Prevention Program of the Population Health Department.

    Mindoula provides 24/7 virtual and in-person support to members who have experienced suicide ideation and/or attempts. Our Care Extenders work to improve their lives and the lives of their families by providing access to appropriate healthcare and community services. This Care Extender (CE) is responsible for understanding and adhering to all contractual requirements established by Mindoula and the client while assisting the team in reaching outcome goals.

    The candidate will have the ability to manage his or her caseload and make decisions without the continuous supervision of the senior management team. The candidate will have access to sensitive patient information and must maintain the integrity of those records at all times. In addition, the candidate must be trustworthy, have a flexible schedule, have excellent organizational skills with a strong attention to detail, be self-motivated, be a team player, and be at ease with a variety of computer programs including electronic health records and G-suite.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    ESSENTIAL FUNCTIONS:

    • Assistance, as needed and requested, with initial outreach and engagement to a cohort of members
    • Continuous outreach and engagement on a regular basis (as contracted)
    • Develop and coordinate community resources with emphasis on medical, behavioral, violence\prevention, and social services.
    • Assess members for risk, make quick evaluations for risk, and respond within the parameters outlined within the member's safety plan and perform crisis intervention that ensures the safety of all members.
    • Assess a myriad of options for care including the use of benefits and community resources to develop and update the service plan.
    • Guide and educate members in the development of skills and/or strategies for managing problems and triggers to increase relapse prevention and reduce risk.
    • Encourage members to develop work skills and to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills and develop social relationships.
    • Educate members about mental or physical illness, abuse, violence prevention, medication, and available community and social resources.
    • Facilitate the scheduling of appointments and follow-up service as well as provider appointments
    • to conduct assessments or face to face visits in the community as needed.
    • Monitor, evaluate, and record member's progress according to measurable goals described in the member's service plan.
    • Act as a liaison and member advocate between the member/family, physician, and facilities/agencies.
    • Remind members, as needed, of upcoming appointments and/or missed appointments.
    • Interact continuously with members, family, physician(s), and other providers utilizing clinical knowledge and expertise to lower over-utilization of unnecessary resources.
    • Request consultation and diagnostic reports from network specialists.
    • Report critical incidents and information regarding the quality of care issues.
    • Prepare and maintain all required records, reports, paperwork, and member's diagnostic records, maintaining confidentiality of members at all times. Develop and implement modifications to member's service plan based on clinical experience and knowledge and in response to changes in member's status, as requested and no less than once every three months.
    • Use clear and accurate communication skills (both verbal and written) to facilitate communication with the member, member's family/caregivers, with staff, and the treatment team; produce high quality documentation that enables the member to receive the best treatment possible.
    • Maintain accurate records of all case management activities in mMR.
    • Accurately complete all case notes in a professional, timely manner.

    REQUIRED EDUCATION AND EXPERIENCE:

    • Bachelor's degree in psychology, social work, or other applicable behavioral health discipline from an accredited college or university is required.
    • Master's Degree in psychology, social work, or other applicable behavioral health discipline from an accredited college or university is preferred
    • Two (2) or more years of direct experience in the behavioral health field
    • Experience working with at-risk adolescents and young adults is preferred. Crisis Intervention Training / Crisis Training
    • Knowledge of local resources, social determinants of health, mental health, substance abuse
    • disorders, interpersonal violence, and social issues.
    • Excellent computer skills and proficiency in Microsoft Word, Google Suite (calendar, docs, sheets, slides etc.), and Excel
    • Strong data entry experience
    • Must be detail-oriented and able to multitask.
    • Must be able to work in a fast-paced environment and have a strong sense of urgency.

    SKILLS REQUIRED:

    • Relentless about prioritizing and following through on assigned tasks independently.
    • Flexibility and adaptability in order to support patients is a must.
    • Superior time management and organization to complete daily tasks by the end of each day
    • Proficiency in navigating the Internet.
    • Proficient in Microsoft Excel skills including edit, search, sort/filter, and format using already created pivot tables to locate information.
    • Proficient in data entry/database management.
    • Compliance: Ensures care is delivered according to applicable laws, regulations, policies and procedures, supporting the organization's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperates fully with all organizational investigations and proceedings. Ensuring health information management requirements are met.
    • Ability to exercise strict confidentiality and sound judgment with respect to sensitive member information, maintaining the integrity of member records at all times.
    • Relationship Management: develop, maintain, and strengthen partnerships with others inside or outside the organization.
    • Problem Solving: Identifies needs and takes independent action to implement change when and where it is needed; results oriented. Ability to plan, organize, manage time, and prioritize multiple tasks and assignments. Use effective follow through.
    • Communication: Excellent listening, verbal, and written communication skills. Clearly and accurately, relaying information to staff, business partners, stakeholders, and members including collaborating with interdisciplinary teams including psychiatrist
    • Ability to work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness.
    • Ability to provide case management in routine and crisis situations.
    • Excellent attention to detail and organizational skills.

    POSITION TYPE/EXPECTED HOURS OF WORK: Full-time Regular position. Flexibility in hours. Some evening work. This position will require the ability to work various schedules when necessary, including outside of “core” work hours and will allow for some flexibility in scheduling as approved by the manager in order to support members' needs.

    TRAVEL: 50% (variable to change). Reliable transportation required.
