Housing Voucher Social Worker - MNVEST
__JOB DESCRIPTION:
The Minnesota Assistance Council for Veterans (MACV) has been awarded the Minnesota Department of Veterans Affairs (MDVA) Minnesota Veterans Entering Stable Tenancy (MNVEST) Subsidy program. MNVEST provides a rental subsidy and clinical case management services to Veteran households experiencing homelessness. The MNVEST Social Worker will provide intensive case management services to Veterans and their families receiving vouchers to exit homelessness. Primary Job Responsibilities/Duties: Conducts client assessments (including financial, personal, and crisis) to identify issues/barriers and create a housing stability plan (HSP) with the Veteran to address the needs defined by the assessment.
PRIMARY DUTIES AND RESPONSIBILITIES
Perform intakes that include required documentation for admission, referral needs, and explanation of program and expectation.
Develop a housing stability plan with the Veteran/household which includes short-term and long-term goals with objectives and update case plan as required.
Provide virtual and in-person case management from a harm-reduction, strength-based model.
Provide financial assistance in accordance with voucher program policies and procedures.
Develop a discharge plan with participants addressing permanent housing stability, economic stability, emotional stability and self-determination.
Assess Veterans and connect participants to necessary services by coordinating with other service providers to ensure access, eligibility and follow-through by participant and provider.
Conduct ongoing evaluations of the participant’s progress through the plan.
Maintain strong working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.
Work with other program staff to ensure compliance with program outcomes and requirements.
Work with partner agencies to review eligibility and support program implementation to include monitoring visits and quality control.
Use Salesforce to track participant and program progress, maintain up-to-date case files and case notes, and ensure programmatic compliance.

LSW minimum requirement
Strong communication skills to communicate directly with Veterans and community providers
Strong problem-solving skills and ability to work under tight timeline toward crisis resolution for Veterans seeking assistance in crisis situations
Two plus years’ experience of working with vouchers and/or subsidized housing and in navigating permanent subsidy programs, or in housing case management working with subsidies and waitlists preferred.
Experience working with individuals who are homeless, chemically dependent or have a mental illness desired.
Veteran, or knowledge of veteran issues, preferred.
SALARY RANGE: $52,000 - $60,000
