We are Mission Connection Healthcare, and we are launching a Virtual Mental Health intensive outpatient program for residents of California & Washington State. JOB DESCRIPTION: The Mission Connection Case Manager is a multifaceted role. The Case Manager is a frontline staff conducting clinical and administrative processes for Mission Connection, and whose primary task is to ensure the smooth and continued operation of the program, whilst simultaneously working to improve the program and enhance the client experience. QUALIFICATIONS: Must be 18 years or older to apply for this position. Masters degree or higher in Psychology, Counseling, or a related/applicable field preferred Associate or licensed MFT, PCC, CSW or PsyD/PhD, with active registration with the CA Board of Behavioral Sciences or other applicable oversight agency. Minimum: 2 years experience working with clients with primary mental health disorders, as well as some experience in administrative processes and clinical leadership experience Team Fit Criteria: The Mission Connection team is Flexible, Dynamic, and Passionate. MUST have proficiency in utilizing Google Workspace applications, specifically Google Calendar, Sheets, Docs, Gmail MUST have proficiency in navigating Zoom MUST have proficiency in utilizing the medium of videoconferencing to provide quality virtual therapeutic services. DUTIES AND RESPONSIBILITIES: The Mission Connection Clinical Coordinator will be assigned projects, based on his/her level of expertise, that include any, or all the following job responsibilities. Oversee and manage client caseloads in the Mission Connection Virtual Program Complete client intakes virtually. Ensure clients meet billing standards. Maintain the record of care in all clients EHR's in accordance with company policy, Joint Commission accreditation requirements, and CA-DHCS guidelines. Work closely with the Clinical Director to ensure that all clients are meeting their clinical objectives. Oversee that the intake and administrative documentation for the client census and see that related documents are accurate and consistently up to date. Arrange and assist in new admissions, specifically in helping to schedule the intake if needed, and scheduling and/or completing initial assessments for clients. Assist the Clinical Director in closely monitoring clients that are heightened suicide risks, maintaining and writing safety plans and administering the CSSRS weekly and lifetime assessments as needed. Provide clinical services for the program, including but not limited to: Group services as needed to maintain coverage for the program. Provide individual therapy as needed and per program guidelines for clients. Assist in the discharge planning process by liaising with A Mission for Michael staff members to inform potential clients of the program rules and regulations. Participate in hiring and recruiting, as well as actively enhancing and formulating program rules and regulations. Serve as the foundation for the program, coordinating between all participants, both clients and staff, communicating regularly with leadership and interdepartmentally throughout the wider organization.
