Physical Therapist Job Description
Essential Duties/Responsibilities
(1) The physical therapy service shall provide:
(A) Muscle, nerve joint and functional ability tests.
(B) Treatment to relieve pain, develop or restore function.
(C) Assistance to achieve and maintain maximum performance using physical means such as exercise, massage, heat, sound, water, light or electricity.
(2) Provide an evaluation by the physical therapist and establishment of a treatment program. The treatment program shall be modified as needed based upon subsequent reevaluations.
(3) Require that physical therapists:
(A) Record treatments, each day, in the participant's health record. Each entry shall be signed.
(B) Record quarterly progress notes. Each entry shall be signed and dated.
(C) Review and initial all evaluations and the discharge summary .
(4) The physical therapist shall accomplish those tasks required by the individualized plan of care within the staffing requirements stated in Section 54423.
(5) The physical therapist shall act as a member of the assessment team in an evaluation of the patient's rehabilitation potential.
(6) Physical therapy procedures may include:
(A) Therapeutic exercise.
(B) Neuromuscular reeducation.
(C) Rehabilitation services.
(D) Functional activities.
(E) Gait training.
(F) Orthotics training.
(G) Prosthesis training.
(H) Massage.
(7) When staff observe or report PT related changes in the Participant, the PT will be responsible to communicate these observations to the Primary Care Provider, Administrator or Program Director to decide if it requires further follow up or an incident report using the Title 22 policy and procedures.
Qualifications
According to Title 22 CCL regulations(Section 54423).
Requirements
Must complete Health Screening Report upon hire (includes proof of TB clearance)
Fingerprints must be submitted upon hire and pass criminal clearances background check
Must provide copy of license/certification and proof of qualifications/education
Job Type: Part time: At least 18hrs a week
