    Job Title: DIRECT SUPPORT PROFESSIONAL I/II, DAY PROGRAM


    Under the supervision of the Director and Program Coordinator of Day Services, this position is responsible for assisting adults with developmental disabilities in promoting their independence in social and employment settings.

    ESSENTIAL FUNCTIONS:

    • Provides on-the-job training to program participants in groups and/or individually
    • Responsible for providing skill or developmental training in various activities as designated; assists with personal care (including toileting/changing)/hygiene
    • Maintains accurate and current records such as daily participant documentation
    • Contributes to positive relations between community members, program participants, vendors, and other Agency staff
    • Trains program participants in accessing public transportation and assists them with community integration
    • Monitors, tracks, and assists program participants in goal achievement
    • Organizes social and vocational groups in promoting independence
    • Complies with all Momentum Inc. program policies, CARF accreditation standards, TCRC, CAL/OSHA policies and procedures
    • Must be able to work at other Momentum WORK, Inc. locations, when needed
    • Other related duties and responsibilities on an as needed basis

    PHYSICAL REQUIREMENTS

    • Ability to stand, sit for long periods of time
    • Ability to go up and down stairs
    • Ability to walk for long periods of time
    • Ability to push or pull wheelchairs when necessary
    • Ability to speak, read, hear, and understand technical written materials
    • The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
    • Full awareness of environmental stimuli
    • Ability to implement Crisis Management, Safety & Pro Act techniques (training included)

    QUALIFICATIONS:

    • High school diploma or equivalent
    • At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
    • Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
    • Excellent verbal and written communication skills
    • Ability to complete tasks with numerous interruptions
    • Compassion and respect for persons with disabilities
    • Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations
    • May need to be available during any disasters, per disaster plan
    • Must have reliable phone
    • Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy
    • Valid certification in CPR/First Aid (Training included)
