Moms In Motion has an immediate opening for a Case Manager DC.

Work from your home office. Must live in Virginia.

The Case Manager DC is responsible for supporting the team and the families we serve by correctly completing an assessment, plan of care, updated outcomes, supervision requests and justifications for each routine, waiver transition, reauthorization, and 6-month reassessment for the ccc+, OWD, CL/FIS, and EPSDT recipients.

Skills and Qualifications include:

Requires a documented combination of skills/relevant work experience. Person-Centered Thinking/Planning training is a plus.

Knowledge of Virginia Medicaid Waivers a plus

Highly organized

Work remotely with electronic devices and associated tools and equipment required to complete day-to-day tasks.

Maintains experience in a variety of applications and programs; not limited to but including Office, Google, & Adobe.

Must maintain a steady and reliable internet connection.

Have good written and verbal communication skills.

Submit to a criminal background check.

Two references are required.
