This is a care management position, in which Care Managers – RNs will create and administer care plans, rather than render direct clinical services. Care Managers – RNs lead the effort to help prevent costly and traumatic episodes such as avoidable hospitalizations, readmissions, and unexpected kidney failure.

For those already on dialysis, Care Managers – RNs provide additional support, particularly around transitions in care such as hospital discharges.

Qualifications

Required

• Valid Registered Nurse (RN) license/certification

Preferred

• 2+ years experience in any of the following:

  • Case Management (CM)

  • Dialysis Care

  • Home Health Care

  • Hospice

Schedule: Full-time Mon - Fri traveling to patients' homes who suffer from chronic kidney disease Ability to take calls remotely on some nights and weekends.

 However, there could be exceptions where a patient does request a visit after 5 pm. Candidates must be within 15 miles of Boston Metro.
