    UNIT MANAGER JOB DESCRIPTION-SCHAUMBURG IL

     

    DESCRIPTION:

     

    The Unit Manager will be responsible for coordinating the care and services provided within the unit in accordance with established policies and procedures. The Unit Manager is a mentor and role model responsible for demonstrating understanding, patience, tact, and courtesy in dealing with associates, residents, families and visitors.

     

    RESPONSIBILITIES:

     

    LEADERSHIP

    • Incorporates the Core Values into job performance to achieve the organization’s mission.

    • Participates in the organization’s strategic planning efforts and operational improvement initiatives to drive innovation and quality of the Unit.
    • Responsible for the implementation of Service Lines, such as Pulmonary Rehab and Cardiac Care to enhance quality of care, reduce hospital readmissions, increase physician and hospital referrals and ultimately admissions to SNF.
    • Responsible for EMR, Risk Watch, and OnShift.

     

    STAFF MANAGEMENT, TRAINING AND DEVELOPMENT

    • Responsible for interviewing, hiring, orienting, and evaluation of the Unit personnel to develop and maintain a resident centered care philosophy.
    • Maintains positive environment using effective problem solving strategies and through staff empowerment to promote high staff morale and retention.
    • Employs communication vehicles to ensure staff is aware and knowledgeable regarding organizational news, changes and needs.
    • Participates in staff development and in-service education of personnel.
    • Completes performance appraisals for staff.

     

    CONTINUOUS IMPROVEMENT

    • Monitor, evaluate, and improve the quality of care and services delivered within a quality and safety culture.
    • Continuously seeks out opportunities to improve both the clinical and operational needs of the Unit.
    • Supports and implements clinical improvement initiatives and ensures a safe and clean work area.

     

    REGULATORY COMPLIANCE

    • Maintains knowledge of current long-term care regulations and programs in accordance with local, state and federal guidelines.
    • Ensures staff compliance with established policies, procedures and regulatory programs in accordance with local, state and federal guidelines.
    • Investigates any/all unusual or abnormal incidents, ensuring accuracy and completeness of incident reports and reports findings to Director of Nursing or designee.
    • Initiates immediate investigation and reporting of complaints, allegations of abuse or neglect and misappropriations of resident property and reports to the Administrator and Abuse Prevention Coordinator.
    • Ensures Medicare Certifications are completed and signed by the attending physician in accordance with Medicare guidelines.

     

    SUPERVISION OF NURSING CARE AND DOCUMENTATION

    • Responsible for ensuring the Unit is properly staffed according to resident acuity.
    • Supervises and holds staff accountable for implementation of policies and procedures and standards of care.
    • Conducts rounds to monitor resident acuity and assess residents’ physical and psychosocial status.
    • Performs audits of residents’ medical records to ensure compliance with facility policies as well as State and Federal regulations.
    • Provides assistance with admissions, discharges and hands-on clinical care as needed to ensure a smooth flowing unit.

     

    CONTINUUM OF CARE

    • Conducts Care Plan meetings with family and residents.
    • Responsible for oversight of discharge planning protocols to ensure timely, safe and smooth resident discharges to the right level of living.
    • Facilitates communication across continuum to ensure delivery of resident-centered care philosophy.
    • Communicates effectively with residents, family members, physicians, other healthcare providers and community constituents.
    • Maintains EMR plan of care.
    • Maintains and reads all resident EMR/paper documentation by staff.
    • All other duties as assigned by supervisor.

    ENVIRONMENTAL AND PHYSICAL DEMANDS:

     

    PHYSICAL ACTIVITIES:

    • Frequently walking, sitting, talking, hearing, reaching, feeling, grasping, crouching, crawling, kneeling, repetitive movement of writing and use of the computer.
    • Occasionally standing, squatting, bending, and twisting.

     

    RANGE OF MOTION:

    • Frequently being able to move from shoulder and above, waist-shoulder, and waist and below height.

     

    QUALIFICATIONS/REQUIREMENTS:

     

    Education/Certification Requirements:

     

    LICENSURE

    • Current RN licensure with the State of Illinois

       

      EDUCATION

    • Graduated from accredited registered nurse program

    • Current CPR card

    • BSN Preferred

     

    Experience Requirements:

    • Minimum one year Long Term Care supervisory experience.

    • Clinical experience in coordination for residents in Medicare/Short Term Rehabilitation unit.

    • A self-directed and motivated individual who is committed to quality resident care and outcomes and who genuinely enjoys actively participating in the formulation and implementation of quality assessment and improvement strategies and goals.

       

      KNOWLEDGE, SKILLS & ABILITIES:

    • A customer focused and directed style that clearly welcomes meeting and exceeding customer expectations for service quality.

    • Strong clinical skills as well as possesses knowledge of nursing theory, practice and administration and someone who can act as the clinical liaison with SNF medical staff.

    • Possess excellent verbal and written communication skills.

    • Proficiency in MS Office (Word, Excel, Outlook)

    • Ability to thrive in a team environment and enjoys staff development and mentoring.

       

       

      OTHER QUALIFICATIONS:

    • Exposure to communicable diseases.

    • Must be able to work with ill, disabled elderly, emotionally upset, and at times, hostile people within the facility.

    • Ability and willingness to work more than usual hours when needed, possibly during a different shift, weekends or holidays.

    • Capable of maintaining regular attendance.

    • Meet all local health regulations and pass post offer pre-employment physical exam, if required, which may include drug screening, criminal background investigation and reference inquiry.

    • Capable of performing the responsibilities and essential job functions with or without reasonable accommodation.

    • Basic level computer skills.

    • Maintain confidentiality of information.

    On occasion, this position allows removal of PHI from FVS campus for purposes of work, in accordance to the Confidentiality Policy
