    General SummaryAssesses nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counseling. The Dietitian performs a variety of duties related to the nutrition counseling of patients/families diet education. This may include but are not limited to diabetes education, weight management, malnutrition/weight gain, infants with failure to thrive, sports nutrition and healthy diet. The Dietitian works as a member of the care team. There is an ongoing interaction of team members to discuss roles, responsibilities, communication and patient care coordination among team members, working together to provide and enhance the care provided by the practice.Essential Job Functions

    Consult with health care personnel to determine nutritional needs and diet restrictions of client.
    
    Evaluate the health and diet needs of clients.
    
    Develop nutrition plans and implement interventions based on knowledge of client’s current health.
    
    Implement personalized, age-specific and culturally appropriate nutrition strategies
    
    Provide nutrition education and counseling for clients.
    
    Develop dietary plans to meet the nutritional requirements of clients.
    
    Supports the patient-centered medical home (PCMH) by educating patients, families and caregivers about nutrition. 
    
    Advise clients and their families on nutritional principles, dietary plans and diet modifications, and food selection. 
    
    Plan and conduct training and education programs in dietetics and nutrition.
    
    Makes appropriate referrals to other services.
    
    Participates in MCHC and community committees as assigned.
    

    Other Responsibilities

    Maintains regular interaction with assigned Program participants to promote positive communications regarding Program issues.

    Attends required meetings and trainings as defined by MCHC and/or CAD/CAH/CAP.

    Other job related duties as assigned.

    QUALIFICATION STANDARDS (EDUCATION, EXPERIENCE AND SKILLS)

    Registered Dietitian or nutritionist must be licensed by the Commission on Dietetic Registration (CDR) or registered and ready to take the exam.

    Master’s degree in Nutrition preferred or related field with a minimum of 3 years experience in some phase of nutrition education , or management of a nutrition program; or Bachelor’s Degree in Nutrition or related field with a minimum of 3 years of experience in some phase of nutrition education, or management of a nutrition program.

    The requisite post Bachelor’s (Master’s) professional work experience must include one year working with children and adolescents.
    
    Demonstrated ability to plan, implement and evaluate programming targeting high-risk clients.
    
    Experienced in culturally sensitive issues relating to the needs of high-risk children, youth and families.
    
    Must have a high degree of communication and interpersonal skills to effectively interact with children, adolescents, parents, neighborhood groups, school staff, community agency personnel, administration, associates and visitors.
    
    Computer Skill Level Required
    
    Basic – Microsoft Outlook
    
    Intermediate – Microsoft Word (Minimum 35 WPM), Excel	 
    
    CPR certification required. A current associate, who does not have a CPR certification will be provided training and certification.
