    ABOUT MOVING ANALYTICS

    Moving Analytics provides Movn - a digital cardiac rehabilitation service to help patients recover after a heart attack or heart surgery. The goal of the program is to help patients reduce their risk of getting a future cardiac event by empowering them to manage their risk factors including exercise, nutrition, smoking, blood pressure, weight, medication and stress.

    BACKGROUND

    Moving Analytics is looking for a Program Coordinator who will identify and enroll eligible patients to Movn. As part of enrolling patients, you will transfer them into the care of their remote cardiac rehab coach.     • Irvine, CA
    • Standard Schedule: a minimum of 3 days on-site is required, with the option to remote 2 days each week

    SCOPE OF WORK

    • Apply a demonstrable, working knowledge of cardiovascular disease (CVD), vitals, risk factors, and common treatments to daily tasks
    • Closely interpret and analyze, as well as connect key/relevant events, in electronic medical records to qualify a patient for remote cardiac rehab
    • Understand the interactions between cardiovascular disease & other comorbidities and conditions to determine patient safety and qualify a patient as being low, moderate, or high risk
    • Use clinical judgment/critical thinking strategies beyond referring to an eligibility criteria list
    • Offer Movn to patients over the phone, explain all program components, and address any questions or concerns
    • Identify perceptual barriers to lifestyle change and implement strategies to encourage behavior modification
    • Onboard patients by familiarizing them with program tools, assisting in both device and app set-ups, and scheduling appointments within the program (e.g., first call with coach, outpatient evaluation appointment(s))
    • Exhibit empathy, attentiveness, patience, and respect in all patient interactions and show sensitivity towards personal health experiences
    • Uphold our standards of, and abide by the HIPAA regulations for, patient privacy
    • Maintain enrollment rates and other measurements of success for the Program Coordinator, as well as work with the team to improve enrollment processes
    • Complete all required and relevant documentation and maintain consistent communication with other key partners involved in the patient's enrollment (e.g., Program Coordinators, coaches, healthcare providers, etc.)
    • Attend and participate in required weekly team meetings held both in-person and through video conferencing
    • Ability to work independently as well as welcome and contribute to team interactions

    MINIMUM REQUIREMENTS

    • Master's Degree in Exercise Science, Exercise Physiology, Nursing, or a related allied health field
    • Phase I or Phase II cardiac rehab experience or relevant internship experience
    • Knowledge about cardiovascular disease and cardiac events (e.g., chest pain, AMI/heart attack, sudden cardiac death), vitals (e.g., blood pressure, heart rate, SpO2), risk factors, and common surgeries, intervention procedures, and treatment plans (e.g., CABG, PTCA, stenting, pharmacological therapy, cardiac rehabilitation) in the acute and long-term phases
    • BLS and/or CPR certification by the American Heart Association/American Red Cross
    • Medical Terminology course
    • Experience managing tasks of varying degrees of complexity simultaneously
    • Tech-savvy with a working ability to troubleshoot technical issues
    • Clear and concise written and verbal communication

    WHAT WE'RE LOOKING FOR

    • ACSM certification (e.g., ACSM Certified Clinical Exercise Physiologist, ACSM Certified Exercise Physiologist, ACSM Certified Personal Trainer)
    • Proficient in electronic medical record systems (e.g., EPIC) 
    • Previous experience working directly with patients
    • Previous experience in clinical documentation and working with provider offices/requesting medical records
    • Previous experience in customer support
