We are seeking a Community Support Specialist to join our Community Inclusion team! You will take part in and assist with various community-based activities Monday - Friday (8:00A - 4:00P).

Responsibilities:

Attend activities with clients, providing support as needed

Administer medications

Provide transportation with company and personal vehicles

Collaborate with clients and co-workers for best care opportunities

Communicate ongoing care results and updates

Implement personalized habilitation plans

​Qualifications:

Previous experience in personal care or other related fields

Compassionate, patient and caring demeanor

Ability to build rapport with clients

Excellent written and verbal communication skills

Must have a reliable vehicle, valid driver’s license and insurance

Must be able to lift 45lbs
