    Job Description

    The Employee Health and Wellness Nurse functions as a member of the human resource and quality departments, with responsibilities in the areas of pre-employment, credentialing, and annual health assessments of employees and volunteers, education, health, and wellness activities throughout all NATIVE HEALTH facilities. Maintains and updates all employee health records and related policies as required by state and federal regulations and coordinate activities related to Workers Compensation. This position will also be responsible for the agency’s infection control program and will be responsible for the development and implementation of Infection Control protocols as needed.

    KEY ACCOUNTABILITIES:

    1. Onboarding Health Screening, Immunization and Testing
    2. Supports administration with employee health databases, maintains complete and accurate employee health records.
    3. Ensures the infection control program is compliant with state and federal requirements, accreditation bodies (AAAHC), and employee health related policies and clinical responsibilities for employees.
    4. Coordinates the development and implementation of all employee health initiatives and training programs.
    5. Supports the organization as needed with other duties as assigned.

    QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

    1. Graduation from an accredited school or college of nursing is required.
    2. Licensed to practice nursing in the state of Arizona as a Licensed Practical Nurse is required.
    3. Certification in Infection Prevention and Control or similar recommended.
    4. At least three (3) years of nursing experience. One or more years in a supervisory position is preferred.
    5. Must have a valid and unrestricted Arizona driver's license (within 30 days of hire if the applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years.  No more than two at-fault accidents in the previous three years.  Must maintain a valid AZ driver's license during employment.  Must be 25 years and insurable by NATIVE HEALTH liability auto policy.
    6. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment.
    7. Must pass drug and alcohol testing upon employment and random testing during employment.
    8. Must have updated Immunization (IZ) record. 
    9. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
    10. Must obtain a CPR Certificate within ninety (90) days of employment and maintain a valid card during employment.

    5. Knowledge of primary care nursing practice, lab testing, phlebotomy, and administration of immunizations.
    6. Knowledge of accreditation and credentialing requirements.

    9. Open to change and willing to lead change within the organization.
    10. Professional integrity; excellent work ethic.
    11. Able to manage competing priorities, excellent organizational skills, and good time management.

    13. Proficient with computer-based applications such as MS Office, Adobe, registries/medical databases, and electronic health record.
    14. Knowledge of community resources and infectious disease protocols.
    15. Must have a sincere desire to improve the health status of the Native Health Employees.
