    Seeking Full-Time RNCM for our fast growing Hospice Agency in Orange County. Hospice Experience preferred

    We are hiring a Registered Nurse Case Manager for our growing Hospice to join our vibrant team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients. Hospice experience is desired. You will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs and provides skilled nursing hospice and palliative care services provided to our patients. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with EMR.

    QUALIFICATIONS: Current RN license within the State. Hospice experience preferred

    Responsibilities

    Monitor patient’s condition and assess their needs to provide the best possible care and advice

    Observe and interpret patient’s symptoms and communicate them to physicians

    Collaborate with physicians and nurses to devise individualized care plans for patients

    Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts

    Adjust and administer patient’s medication and provide treatments according to physician’s orders

    Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.

    Supervise and train LVNs and Home Health Aides

    Foster a supportive and compassionate environment to care for patients and their families

    Expand knowledge and capabilities by attending educational workshops, conferences etc.

    Skills

    Proven experience as a registered nurse

    Knowledge of end-of-life care and procedures

    In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times

    A team player with excellent communication and interpersonal skills

    Responsible and compassionate

    Outstanding organizational and multi-tasking skills

    Patient with excellent problem-solving skills

    BSN or other diploma from a nursing program

    Valid nursing license
