    NITELINES USA IS CURRENTLY LOOKING TO FILL SEVERAL COUNSELOR / SOCIAL WORKER POSITIONS AS LISTED BELOW:

    POSITION: COUNSELOR – SOCIAL WORKER

    STATUS: FULL-TIME

    LOCATION: 633 Indiana Avenue NW, 2101 Martin Luther King Avenue SE, or any CSOSA facility in Washington, DC  20004

    HOURS OF OPERATION. CSOSA normal hours of operation are Monday through Friday, 8 AM to 5 PM.

    OBJECTIVE: Function as behavioral healthcare system navigators for offend. The Counselor-Social Workers will work as part of an interdisciplinary team with assigned offender’s Community Supervisor Officer (CSO) and Intervention Specialist (IS) and will participate in case planning, case plan updates, and direct contact with the offender to navigate attendance and participation in CSOSA programs, or services provided by community-based mental health or other service providers.

    REQUIREMENTS:

    Each Counselor-Social Worker will be assigned up to 50 offenders and shall perform the following duties:

    • Collaborate with CSO’s and IS’s to coordinate care management, facilitate the completion of relevant releases of information necessary to facilitate care, and develop treatment plans and goal planning for offenders with substance use and other behavioral health needs.
    • Assess offender needs, identify community resources, link offenders to community resources, including local government agencies, and collaborate with community resources.
    • Develop and update comprehensive, collaborative care plans, based on provider treatment plan, evidence-based chronic care guidelines, and offender/family goals for offenders.
    • Assist with, and facilitate, the transition of care from inpatient or community-based settings.
    • Provide individual offenders/family education and self-management support that is appropriate based upon the offender’s language, cognitive abilities, literacy level, learning style, cultural norms, offender preference, readiness for change, and available resources.
    • Address medication management.
    • Educate offenders regarding health conditions.
    • Communicate changes in offender status appropriately with the care team.
    • Perform an after-visit summary review with offenders, when appropriate.
    • Identify barriers when treatment goals are not met, treatment plan is not being followed, or important appointments are missed.
    • Identify offenders who are overdue for visits, labs, and referrals. Contact offenders and arrange for follow-up services.
    • Assist the offender in improved healthcare access, and promote offender knowledge of health and behavior change.
    • Facilitate the initiation of diagnostic services, treatment planning, and therapeutic treatment.
    • Utilize appropriate protocols and standing orders in delivering offender care.
    • Assist offenders in registering for, and obtaining, healthcare insurance, and communicate with insurance companies or third-party payers (such as Medicare or Medicaid) for offenders.
    • Assist offenders with medical appointments and group visits.
    • Participate in regular team meetings, progress review meetings, compliance review meetings, discharge staffings, and other necessary staffings and meetings.
    • Consult with the medical staff, nursing staff, and ancillary department staff (such as the District of Columbia, Department of Behavioral Health Core Service Agencies) to eliminate barriers to the efficient delivery of care.
    • Identify service delivery problems and potential for effective offender management intervention.
    • Document all communication with offender and upload documents in designated CSOSA offender case management system(s).
    • Work at any CSOSA office location, or remotely, as directed.

    GENERAL QUALIFICATIONS: 

    • Be a citizen of the United States of America, or be lawfully permitted to reside in
    • the United States and possess a valid work permit.
    • Be at least 21 years of age, and must be able to withstand the physical demands of the
    • job, and be capable of responding to emergency situations.
    • Be able to clear the CSOSA background investigation.
    • Be able to speak English fluently, and read and write the English language.
    • Maintain current, full, and unrestricted licensure, where applicable.

    SPECIFIC QUALIFICATIONS: AT A MINIMUM, THE COUNSELOR-SOCIAL WORKER SHALL POSSESS:

    • Master’s degree in social work from an accredited college or university required.
    • One year of experience working with behavioral health offenders.
    • Licensure* as a Licensed Clinical Social Worker (LCSW), Licensed Professional
    • Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Registered Nurse (RN), or Advance Practice Registered Nurse (APRN). * Requires a full, current, and unrestricted license to practice in the District of Columbia, or a state, territory or Commonwealth of the United States.
    • Current CPR certification required.

    Demonstrated skill in:

    1. Individual care/treatment plan development, review, and update (required).
    2. Discharge planning (required).
    3. Assisting with, and facilitating, transition of care from inpatient to community based settings (preferred).
    4. Behavioral health screening and assessment (preferred).
    5. Identifying and linking offenders to community resources (preferred).
