    General Summary of Job Duties and Responsibilities : The Community Health Worker (CHW) for NOVUS Health is responsible for helping patients and their families to navigate and access community services, other resources (including Medicaid/ACA education and enrollment) and adopt healthy behaviors. In addition, the CHW will support providers and case managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their families. The CHW provides social support and informal counseling and advocates for individuals and community health needs.

    ** Essential job functions:**

    Outreach/Community Activities Duties:

    • Provide community outreach and health educational programs to targeted populations, including participation in health fairs and community events.
    • Recruit/refer new patients to NOVUS Health for healthcare and other services.
    • Assist in the planning and coordination of community health fairs and events.
    • Assist in recruiting and coordinating outreach volunteers.
    • Represent agency at community events as directed or needed.
    • Serve as the liaison to local, state and national CHW organizations.

    Patient Care Duties:

    *Provide follow-up, basic motivational interviewing and goal setting with patients/families and provide referrals as necessary.

    • Act as a patient advocate and liaison between the patient/family and community service agencies.
    • Complete PrEP and STI training, may assist with PrEP navigation as needed.
    • Serve as liaison between patients and pharmacy as needed.

    Promotion and Marketing Duties:

    • Promote NOVUS Health, particularly programs and services throughout the community.
    • Assist and participate in social media and printed marketing, providing feedback or design ideas.

    Health Insurance and Medicaid:

    • Complete annual CMS Certified Application Counselor training and maintain Missouri Navigator License.
    • Provide education about and enroll individuals into the Missouri Medicaid or Affordable Care Act plans.
    • Assist uninsured patients with options including sliding fee scale.

    General Duties:

    • Maintain comprehensive documentation for data collection and/or reports, including recording patient encounters in EMR no later than 24 hours after contact.
    • Participate in Staff meetings and other meetings as assigned.
    • Exhibit excellent working relations with patients, visitors and staff.
    • Effectively communicate NOVUS Health’s mission.
    • Knowledgeable about community resources appropriate to needs of patients/families.
    • Maintain HIPPA compliance.
    • Other duties as assigned.

    Education Requirements:

    • High school graduate or equivalent required; Associate degree in healthcare or related field preferred.
    • Successful completion of a Community Health Worker formal training program from a college or other education institution is required.
    • Medical terminology and/or background preferred.
