  • Responsibilities

    We are seeking a compassionate and collaborative advanced practice professional to join a patient-focused care team in the Johnstown, PA. In this role, you will provide high-quality clinical care, support positive patient outcomes, and work closely with physicians, nurses, and other healthcare professionals to deliver an exceptional care experience.

    Responsibilities

    • Perform comprehensive patient assessments, including history taking and physical examinations.
    • Diagnose, treat, and manage acute and chronic health conditions within scope of practice.
    • Order, review, and interpret diagnostic tests and laboratory results.
    • Develop and implement individualized care plans in collaboration with the healthcare team.
    • Prescribe medications and other treatments as appropriate and permitted by licensure.
    • Educate patients and families on prevention, treatment plans, medications, and follow-up care.
    • Document patient encounters accurately and maintain timely, thorough medical records.
    • Support continuity of care through appropriate referrals and coordination with specialists when needed.
    • Participate in quality improvement efforts and follow clinical protocols and best practices.

    Qualifications

    • Current licensure as a Nurse Practitioner or other applicable advanced practice credential in Pennsylvania.
    • National certification in the relevant specialty area, if required.
    • Advanced clinical knowledge and the ability to make sound, evidence-based decisions.
    • Strong communication and interpersonal skills with a patient-centered approach.
    • Ability to work effectively in a team-oriented healthcare environment.
    • Proficiency with electronic medical records and standard clinical documentation practices.
    • Commitment to professionalism, confidentiality, and high standards of patient care.

    Preferred Attributes

    • Experience working in a primary care, urgent care, specialty, or hospital-based setting.
    • Familiarity with care coordination, chronic disease management, and preventive care.
    • Comfortable working in a fast-paced environment while maintaining attention to detail.
