    About Us Waterfront Pharmacy is a trusted partner in long-term care, delivering comprehensive pharmaceutical services and clinical support to skilled nursing and assisted living facilities. As part of our commitment to improving resident outcomes and enhancing care delivery, we are expanding our team and seeking a Nurse Consultant to work directly with our partnered skilled nursing facilities.

    Job Summary

    We are seeking a Nurse Consultant to join our team. The Nurse Consultant will support clinical operations at our partner skilled nursing facilities. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

    Responsibilities

    Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans at Skilled Nursing Facilities.

    Administer medications and monitor patients for side effects or reactions

    Document any changes in patient conditions and report on changes

    Maintain accurate records of patient care and discuss observations with the supervisor

    Collaborate with facility Directors of Nursing and Administrators to improve outcomes.

    Should be able to perform med pass reviews and three-way checks.

    Qualifications

    Currently licensed Registered Nurse (RN) in California.

    Previous experience as a Registered Nurse is preferred.

    Minimum of 1 - 3 years experience in skilled nursing or long-term care.

    Excellent interpersonal and client care skills.

    In-depth knowledge of CMS regulations, infection prevention, and QAPI.

    First aid/CPR certification required.

    Ability to travel to facilities as needed.
