    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     POSITION SUMMARY: We have an exciting opportunity to join our team as a Dental School Health Program OUTREACH SPECIALIST.   An Outreach Specialist in the Dental School health program works directly with parents and school staff to increase access to and enrollment in the program.

    JOB RESPONSIBILITIES:

    • Enroll new patients in the Dental School Health Program. Keep record and statistics of all new patients.
    • Act as a liaison between an uninsured patient and the insurance company representative
    • Provide information on the program to parents at PTA meetings, school events and health fairs. These may require flex hours as per when they are scheduled.
    • Work with the Parent Coordinators to increase awareness of the program, enroll students and educate them on all the program has to offer so they will advocate for DSH.
    • Make telephone calls to parents to increase awareness of the program.
    • Work directly with the Dental School Health Outreach Coordinator to improve the outreach process.
    • Attend dental screening events to support the provider that is screening.
    • Prepare outreach material for the schools and oral health screenings.
    • Follow up with the children that are screened and have urgent needs for treatment
    • Work with parent/ guardian one on one to complete the consent for treatment forms (bilingual).
    • Maintain logs/statistics on each schools enrollment. Provide monthly reports.
    • Develop a strategic visit calendar for program schools, under the direction of the Dental Outreach Coordinator, to ensure all schools are visited on a regular basis.

    MINIMUM QUALIFICATIONS: To qualify you must have a High School Diploma 2 years work experience Fluent in Cantonese/Mandarin required

    Must be able/ willing to travel to various offsite locations/schools throughout the school year.

    PREFERRED QUALIFICATIONS: Previous outreach experience
