    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program.     Position Summary:
    We have an exciting opportunity to join our team as a Community Health Worker - Fulltime - Monday - Friday - Brooklyn ( Spanish Speaking) - 9a-5p.

    In this role, the successful candidate The Community Health Worker (CHW) is part of an interdisciplinary team that includes, but is not limited to, medical and behavioral health providers servicing pediatric patients and their families in a primary care clinic setting. We are looking for a motivated, compassionate community member interested in improving the health of their neighborhood, parents, and children.

    Job Responsibilities:

    * Work with a caseload of patients and their families, collaboratively with the clinical team to support each family to reach goals they have set with the clinical team 
    * Assist the Pediatric team to administer and complete early childhood screenings, including social needs, developmental milestones and maternal depression
    * Provide referrals and follows-up, as appropriate, to help families make connections to key community and healthcare resources
    * Facilitate and coordinate Early Intervention referrals
    * Facilitate and Coordinate referrals to the Committee on Pre-School Special Education (CPSE) and referrals to Home Visiting, Early Head Start, Head Start and Day Care programs.
    * Collect data, initiate surveys and maintain logs, as requested, for Early childhood specific projects
    * Maintain detailed, accurate, and confidential documentation of contacts with patients and families using databases like RedCap, FindHelp and electronic medical record (Epic).
    * Provide support and advocacy on early childhood development issues to patients and families 
    * Enroll patients into MyChart
    * Maintain proper communication within department and clinic practice by promptly responding to electronic communication (Email, In-Basket message, Instant Message)
    

    Minimum Qualifications:
    To qualify you must have a High School Diploma or Associates Degree with at least 1 year of experience in a health or education service relate field. Bi-lingual English/Spanish. Computer Skills working knowledge in the use of Microsoft Office.

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $44,274.07 - $46,487.70 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
