    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     Position Summary:
    We have an exciting opportunity to join our team as a Patient Navigator - Fulltime - Brooklyn - 9am - 5 pm - Monday - Friday.

    In this role, the successful candidate The Patient Navigator in HIV prevention coordinates patient care services, including rapid testing, sexual health education, counseling and referral services to patients at risk of HIV/STI transmission or acquisition at Red Hook Center. The Patient Navigator is responsible for carrying out tasks that are needed to execute the HIV prevention plan including medical and social support services.

    Job Responsibilities:

    * Conducts Intakes and Assessments for program eligibility and enrollment.
    * Performs other duties as assigned or volunteered in accordance with the mission, goal, and values of NYU Family Health Centers.
    * Maintains competence through in-service and educational programs.
    * Performs appropriate program documentation of services provided by entering information into all designated databases/EMR systems.
    * Maintains a daily log of patient services for statistic and reporting purposes.
    * Assists in logistics for weekly health education encounters through group/individuals interventions as needed.
    * Educates, coaches, and empowers patients.
    * Provides critical feedback to other members of the health care team, as needed.
    * Provides translation services as needed.
    * Coordinates ongoing navigation and logistical support for linkage and retention of care.
    * Provides benefits navigation, as needed.
    * Provides internal and external referrals to needed social support services in the community.
    * Provides sexual health education and risk reduction counseling as needed.
    * Accompanies patients and/or facilitates linkage and retention to care as needed.
    * Assists with quality improvement projects 
    

    Additional Position Specific Responsibilities:
    Provides HIV rapid testing in assigned hubs as needed.
    Represents the program and/or Family Health Centers at events, as needed
    Travel between FHC locations (Red Hook and Sunset Terrace)

    Minimum Qualifications:
    To qualify you must have a Bachelors degree, or equivalent combination of education and patient navigation/case management experience (e.g., Associates degree and two years experience, high school diploma/GED and four years experience). Past experience and employment in a hospital or health care setting preferred. Fluency in Spanish strongly preferred.

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $38,808.64 - $41,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
