    Position Summary:
    We have an exciting opportunity to join our team as a Community Health Worker - Monday - Friday - 9a-5p - Brooklyn - (Spanish Required).

    In this role, the successful candidate We are seeking a motivated, compassionate Community Health Worker to join the Strengthening Community Mental Wellness (SCMW) initiative. SCMW aims to strengthen the behavioral health system of care in Sunset Park and Flatbush, Brooklyn by enhancing preventive services in the community and in the maternal, infant, and child behavioral health system of care. This innovative initiative, funded by the Robin Hood Foundation, is a collaborative effort between the Family Health Centers at NYU Langone and Together Growing Strong (TGS), an NYU Grossman School of Medicine initiative that aims to strengthen early childhood health and wellbeing by supporting families, providers, and educators in Sunset Park, Brooklyn.
    The community health worker will support participants social needs and wellbeing during their pregnancy and postpartum periods. The CHW will contribute to the well-being and success of participants primarily by recruiting participants, co-facilitating group sessions, conducting assessments, providing case management, and supporting research efforts to evaluate program efficacy. This position offers an exciting opportunity to make a lasting impact on the standard of care and patient experience for women and their families.     Job Responsibilities:

    * Work with a caseload of patients and their families, collaboratively with the social worker and project team, to support each family to reach their goals
    * Assist to recruit and enroll parents/caregivers into the SCMW initiative
    * Assist to administer maternal and early childhood assessments, including social needs
    * Provide referrals and follows-up, as appropriate, to help families make connections to key community and healthcare resources
    * Establish and maintain partnerships with internal stakeholders at the Family Health Centers and external community-based organizations. 
    * Collect data, initiate surveys and maintain logs, as requested
    * Maintain detailed, accurate, and confidential documentation of contacts with patients and families using databases like RedCap, FindHelp and electronic medical record (Epic).
    * Provide support and advocacy on maternal and early childhood needs to participants 
    * Maintain proper communication within department by promptly responding to electronic communication (Email, In-Basket message, Instant Message)
    * Assist with program reporting, evaluation and Continuous Quality Improvement efforts.
    * Facilitate a work environment that promotes wellness, collaboration and professional growth.
    

    Minimum Qualifications:
    To qualify you must have a high school or equivalent with at least 1 year of experience in a health or education service relate field. Bi-lingual English/Spanish. Computer Skills working knowledge in the use of Microsoft Office.

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $44,274.07 - $46,487.77 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
