    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     Position Summary:
    We have an exciting opportunity to join our team as a NYSDOH Preg Prev Prgm Adm.

    In this role, the successful candidate The CAPP/PREP Program Coordinator will be responsible for building relationships with CAPP and PREP implementation sites and scheduling MPC and MAD-CI workshops. They will also will facilitate evidence-based sexual health workshops for adolescents in a high school setting. The Program Coordination will work with CAPP and PREP health educators to ensure required documentation is completed.

    Job Responsibilities:

    Develop relationships with Brooklyn-based high schools, middle schools, and community- based organizations (CBO) to expand Project SAFE’s evidence-based program (EBP) reach
    Work with high school and CBO staff to schedule workshops in the target communities (Sunset Park and Central Brooklyn).
    Facilitate Making Proud Choices workshops with high school students in high schools and CBOs in Brooklyn
    Maintain CAPP and PREP Master Adaptation list
    Work with Program Manager to ensure all EBP participants receive sexual risk assessment screening are connected to risk reduction counseling and appropriate services.
    Work with Program Manager to ensure CAPP staff complete documentation in Online Reporting System (ORS) and efforts to Outcomes (ETO) and provide technical support to
    staff as needed
    Contribute to development of single session workshops
    Work with Program Manager to ensure that a Positive Youth Development Approach is used in EBP workshops and that all services and lessons are LGBTQ+ inclusive
    Conduct observations for CAPP and PREP Health Educators and provide constructive feedback
    Facilitate advanced peer education group at Project Reach Youth (as needed)

    Minimum Qualifications:
    To qualify you must have a High School Diploma required, Bachelors' Degree preferred
    2 years experience in providing health education services with adolescents

    Preferred Qualifications:
    Strong interpersonal skills and organizational skills

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $47,377.20 - $51,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
