Position Summary:
We have an exciting opportunity to join our team as a Care Manager - Monday - Friday - 9 am - 5 pm.
In this role, the successful candidate The RAP Care Manager will manage a case load of HIV-positive patients enrolled in the Retention and Adherence Program (RAP) and participate in all aspects of the program. Primary responsibilities include providing medical case management services which include; treatment education and adherence support, supportive counseling services, on-site case management, linkage and follow-up to on-site and external medical, mental and social services, the delivery of behavioral change interventions and DEBIs, oversight of Peer Educators and co-facilitation of youth and adult support groups. Additionally, the RAP Specialist will conduct intakes into the clinic for patients accessing HIV care. Responsibilities will also include internal data collection, reporting, and analyzing for contractual submission.
Job Responsibilities:
* Provide intake, needs assessment, HIV orientation, service planning and ongoing reassessment services to patients documenting all such services in EMRs
* Screen patients for entitlements eligibility and other support services needs, and if necessary, assist patients in obtaining them through advocacy and navigation
* Coordinate and document case conferencing for each client within 30 days of intake and on a bi-annual basis
* Provide support, crisis intervention, and referrals to emergency services. Refer patients with overt psychiatric manifestations to mental health providers as appropriate
* Maintain appropriate documentation of all client encounters and complete reporting requirements as needed to meet any regulatory requirements
* Maintain accurate, up-to-date records within the Electronic Medical Records and complete additional reporting/tracking requirements as requested
* Conduct Outreach in accordance with the RAP Program and department needs, including promotion of FHC programs and services at health fairs, Pride events, and in the community.
* Follow all internal protocols for services and work toward meeting the targets of contracts managed within the program.
* Data entry and record collection, analysis, and reporting of program services
* Build relationships and ensure collaboration between departments, outside agencies, and community stakeholders
* Represent the program and FHC at and/or attend professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor
* Maintain an appropriate case-load as designated by the program specifications and agency needs
* Assist in program and departmental quality improvement projects
* Perform other duties as assigned by supervisor to maintain the efficiency and effectiveness of prevention and outreach services

Performing job duties may require prolonged periods of sitting standing walking
Care Manager may be required to conduct home visits in the community and must be able to utilize public transportation to access patients homes.
Minimum Qualifications:
bachelors degree
To qualify you must have a 1 year previous case management or navigation experience
Preferred Qualifications:
HIV related work or lived experience. Familiarity working with underserved populations (LGBTQ+, BIPOC, Immigrants, etc.)
Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $48,000 - $55,000 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
