    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     POSITION SUMMARY: We have an exciting opportunity to join our team as a Health Educator at Project Reach Youth (PRY).  In this role, the successful candidate as The Health Educator will facilitate Adult Identity Role Models (AIM) evidence-based curriculum at Erasmus Campus Educational Complex with middle school students as part of the SRAE after school program and sexual health education for high school students and out of school youth.

    JOB RESPONSIBILITIES:

    • Plan and lead trips for participants during school breaks
    • Work with director to prepare biannual reports for SRAE grant
    • Work with program coordinator to provide AIM curriculum, individual meetings with youth, parent workshops and referrals as part of the SRAE program
    • Work with Brooklyn College Community Partnership (BCCP) to ensure all afterschool students have access to SRAE services.
    • Facilitate AIM curriculum with middle school students at Erasmus Campus
    • Build relationships with community partners, school staff and parent organizations
    • Facilitate Making Proud Choices (MPC) curriculum for youth ages 14-24 in partner high schools and CBO
    • Provide individual risk reduction counseling for MPC participants
    • Facilitate single session sexual health workshops for high school students and out of school youth.
    • Use Positive Youth Development Approach and ensure LGBTQ+ inclusivity in all in programming

    MINIMUM QUALIFICATIONS: To qualify you must have a HS Diploma Prior work experience with community health outreach. Health related background.

    PREFERRED QUALIFICATIONS: Strong interest in working with adolescents
